CO and Smoke Detectors: Failure to Alert

Among the most consequential recall categories are life-safety devices that fail at their primary function. A number of smoke and carbon monoxide detector models have been recalled in 2025–2026 for failing to reliably alert consumers to the presence of a fire or CO buildup.

These cases are particularly significant from a product liability standpoint because the harm model is direct and well-established: a detector that fails to alert provides a false sense of security and may delay evacuation. Forensic investigation typically involves electrochemical testing of sensor elements, circuit board failure analysis, and firmware review.

Heated Insoles: Lithium-Ion Battery Failure

A recall of heated insoles documented a failure mode in which the battery can explode and ignite even when the device is switched off. The switched-off ignition failure is notable: it suggests a battery management circuit that does not fully isolate the cell from load in the off state, allowing thermal runaway to initiate from a parasitic drain condition.

Lithium-ion thermal runaway cases are among the most technically intensive product failure investigations — they require cell-level electrochemical characterization, manufacturing quality documentation review, and frequently involve fire debris analysis where the initiating device has been consumed.

Bicycle Helmets: Impact Attenuation Failures

Several bicycle helmet models have failed impact-attenuation requirements in recent CPSC testing. These cases typically reveal failures in EPS foam density consistency, shell-to-liner bonding, or retention system geometry — all of which affect the helmet's ability to manage the deceleration forces of an impact.
